Overview

Pericarditis is an inflammatory condition affecting the pericardium, the thin, two-layered sac that surrounds and protects the heart. When the pericardium becomes inflamed, the layers may rub against each other, leading to chest pain and discomfort. Pericarditis can develop suddenly (acute pericarditis) or persist for several weeks or months (chronic or recurrent pericarditis). While many cases are mild and resolve with appropriate treatment, some may lead to complications if left untreated. Early evaluation by an experienced cardiologist helps identify the underlying cause, relieve symptoms, and prevent long-term heart-related problems.

Causes

Pericarditis can occur due to a variety of medical conditions and infections. Viral infections are among the most common causes, although bacterial, fungal, and other infectious organisms may also be responsible. Autoimmune disorders such as lupus or rheumatoid arthritis, recent heart surgery, heart attack, kidney failure, chest injuries, and certain medications may also trigger inflammation of the pericardium. In some individuals, the exact cause remains unknown, a condition referred to as idiopathic pericarditis. Identifying the underlying cause is an important part of treatment, as it helps guide the most appropriate management plan.

Symptoms

The most common symptom of pericarditis is sharp or stabbing chest pain, which may worsen when lying down, taking deep breaths, coughing, or swallowing and often improves when sitting up or leaning forward. Some patients may also experience shortness of breath, fatigue, fever, weakness, rapid heartbeat, or a general feeling of illness. Depending on the severity and underlying cause, symptoms may develop suddenly or gradually. Since chest pain can also indicate a heart attack or other serious cardiac conditions, prompt medical evaluation is essential for an accurate diagnosis.

Diagnosis

Diagnosing pericarditis involves a detailed review of the patient’s symptoms, medical history, and a comprehensive physical examination. During the examination, the cardiologist may detect a characteristic pericardial friction rub using a stethoscope. Diagnostic investigations commonly include an electrocardiogram (ECG), echocardiogram, chest X-ray, and blood tests to identify inflammation or infection. Advanced imaging studies such as cardiac CT or cardiac MRI may be recommended in selected cases to assess the extent of inflammation, detect fluid accumulation around the heart, and rule out other cardiac conditions.

Treatment Options

The treatment of pericarditis depends on its underlying cause, severity, and whether complications are present. Mild cases often respond well to anti-inflammatory medications that reduce pain and inflammation. Colchicine may be prescribed to lower the risk of recurrence, while corticosteroids may be considered in selected patients when other treatments are not suitable. If the condition is caused by a bacterial infection, appropriate antibiotics are required. In cases where significant fluid accumulates around the heart and affects its function, a procedure called pericardiocentesis may be performed to drain the excess fluid. Patients with recurrent or complex pericarditis may require specialized long-term management under the supervision of a cardiologist.

Post-Treatment Care

Recovery from pericarditis requires careful follow-up and adherence to the prescribed treatment plan. Patients should complete all medications as directed, attend scheduled follow-up appointments, and undergo repeat imaging or blood tests when advised. Strenuous physical activity should generally be avoided until inflammation has completely resolved and the cardiologist confirms that it is safe to resume normal activities. Maintaining a heart-healthy lifestyle, staying well hydrated, managing underlying medical conditions, and promptly reporting any recurrence of chest pain or related symptoms can help reduce the likelihood of future episodes.

Risks or Complications

Although many patients recover completely with timely treatment, untreated or severe pericarditis can lead to serious complications. Fluid may accumulate around the heart, resulting in pericardial effusion, which can progress to cardiac tamponade—a life-threatening condition that requires immediate medical attention. Recurrent episodes of pericarditis may occur in some individuals, while chronic inflammation can lead to constrictive pericarditis, where the pericardium becomes thickened and restricts the heart’s normal movement. Early diagnosis and appropriate management significantly reduce the risk of these complications and improve long-term outcomes.

Medical attention should be sought immediately if you experience sudden or severe chest pain, especially if it is accompanied by shortness of breath, dizziness, fainting, rapid heartbeat, or excessive sweating. Even mild but persistent chest discomfort should never be ignored, as it may indicate pericarditis or another serious heart condition. Patients who have been diagnosed with pericarditis should also consult their cardiologist if symptoms worsen, return after treatment, or if new symptoms such as swelling, fever, or increasing breathlessness develop.
